High School to 4+ Program Transition Rubric
As you consider the learning characteristics of your students, use the following rubric as a guide to determine college success with 4+ support.
|Attendance||Attends regularly with little or no absences and/or tardies.||Attends regularly with few absences and/or tardies.||Does not attend regularly. Has many absences and/or tardies.|
|Self-Advocacy||Appropriately asks for and uses assistance.||Sometimes asks for and uses assistance||Rarely asks for or uses assistance OR is overly dependent upon assistance to complete tasks.|
|Behavior||Follows school behavior policies.||Follows school behavior policies most of the time.||Does not follow school behavior policies.|
|Academics||No or minimal modifications. Most or all of the day is in general education classes.||Few modifications. Moderate accommodations. Majority of the day is in general education classes.||Many modifications or substitutes specially designed instruction for general curriculum.|
With a score of 10-12, the probability of student success with 4+ general curriculum is likely.
With a score of 5-9, the probability of student success with 4+ Career Readiness is likely.
With score of 4 and under, the probability of student success with 4+ support is low.
