High School to 4+ Program Transition Rubric

As you consider the learning characteristics of your students, use the following rubric as a guide to determine college success with 4+ support.

 
Performance Criteria 3 2 1 Score   Comments
             
Attendance Attends regularly with little or no absences and/or tardies. Attends regularly with few absences and/or tardies. Does not attend regularly. Has many absences and/or tardies.    
           
Self-Advocacy Appropriately asks for and uses assistance. Sometimes asks for and uses assistance Rarely asks for or uses assistance OR is overly dependent upon assistance to complete tasks.    
           
Behavior Follows school behavior policies. Follows school behavior policies most of the time. Does not follow school behavior policies.    
           
Academics No or minimal modifications. Most or all of the day is in general education classes. Few modifications. Moderate accommodations. Majority of the day is in general education classes. Many modifications or substitutes specially designed instruction for general curriculum.

With a score of 10-12, the probability of student success with 4+ general curriculum is likely.
With a score of 5-9, the probability of student success with 4+ Career Readiness is likely.
With score of 4 and under, the probability of student success with 4+ support is low.
